摘要:胃癌是我国高发的恶性肿瘤之一,靶向干预是其防治的重要策略。程序性细胞死亡(programmed cell death, PCD)是维持机体稳态并参与肿瘤发生发展调控的关键环节,具有重要的研究与转化价值。本文系统综述了中医药通过调控凋亡、铁死亡、焦亡和自噬等PCD防治胃癌的研究进展。现有研究表明,中医药能够通过多成分、多靶点、多通路的综合调控作用,干预PCD相关通路,从而实现抗癌效应。尽管目前相关证据多来自体内外实验,临床证据仍有待进一步完善,但中医药在调控PCD以防治胃癌方面已展现出独特优势与广阔前景。未来应结合现代科学技术深入阐明其分子机制,并开展更高质量的临床研究,以期为胃癌个体化治疗与新药研发提供更坚实的理论依据与实践指导。
Abstract: Gastric cancer ranks among the most prevalent malignancies in China, necessitating targeted interventions as a crucial strategy for its prevention and treatment. Programmed cell death (PCD) plays a pivotal role in maintaining organismal homeostasis and regulating tumor initiation and progression, thereby possessing significant research and translational potential. This review systematically examines recent advancements in the prevention and treatment of gastric cancer through traditional Chinese medicine (TCM), focusing on the modulation of PCD modalities such as apoptosis, ferroptosis, pyroptosis, and autophagy. Current evidence suggests that TCM exerts anti-cancer effects by modulating PCD-related pathways through integrated actions involving multiple components, targets, and signaling networks. While the majority of existing evidence is derived from in vitro and in vivo experimental studies, and clinical evidence requires further substantiation, TCM has demonstrated distinct advantages and promising potential in the prevention and treatment of gastric cancer via PCD regulation. Future research should employ advanced scientific and technological methodologies to elucidate the underlying molecular mechanisms and undertake higher-quality clinical investigations. This approach will contribute to a more robust theoretical foundation and offer practical guidance for individualized therapy and the development of novel drugs for gastric cancer.
