摘要:目的 采用两样本双向孟德尔随机化(MR)分析,评估膝关节内外翻畸形和膝骨关节炎(KOA)之间的潜在因果关系。方法 本研究使用来源于大规模GWAS荟萃分析的统计数据,主要采用逆方差加权法,协同MR-Egger回归法、加权中位数法、加权众数法、简单模型法来探究膝关节内外翻畸形与KOA之间的因果关联,此外还进行了敏感性分析以检验MR分析结果是否可靠,反向MR分析以同样方法展开。结果 MR分析显示,基于遗传学数据,膝关节内外翻畸形与KOA OR=1.686,95%CI(1.379,2.060),P=3.38×10-7之间存在反向因果关系,敏感性分析也证实了这些关联的稳健性。然而,没有证据表明膝关节内外翻畸形与KOA存在正向因果关联。结论 本研究提供了强有力的证据,表明KOA与膝关节内外翻畸形的风险增加有因果关系,是其发生发展的危险因素,若能有效防治KOA将减少膝关节畸形的发生或进一步加重,未来亟需更多深入的研究来剖析其中的潜在机制。
Abstract: Objective To evaluate the potential causal relationship between genu varus/valgus deformity and knee osteoarthritis (KOA) using a two-sample bidirectional Mendelian randomization (MR) analysis. Methods This study utilized summary statistics derived from large-scale GWAS meta-analyses. The primary analysis employed the Inverse Variance Weighted (IVW) method, supplemented by MR-Egger regression, weighted median, weighted mode, and simple mode methods to investigate the causal association between genu varus/valgus deformity and KOA. Sensitivity analyses were conducted to assess the robustness of the MR findings. A reverse MR analysis was performed using the same methods. Results MR analysis revealed, based on genetic data, evidence for a causal effect of KOA on the risk of genu varus/valgus deformity OR = 1.686, 95% CI (1.379, 2.060), P = 3.38×10??. Sensitivity analyses confirmed the robustness of this association. However, no evidence was found for a causal effect of genu varus/valgus deformity on the risk of KOA. Conclusion This study provides strong evidence that KOA has a causal effect on increasing the risk of genu varus/valgus deformity, identifying it as a risk factor for its development and progression. Effective prevention and treatment of KOA may reduce the incidence or progression of knee deformity. Further in-depth research is warranted to elucidate the underlying mechanisms.
