Bidirectional Two-Sample Mendelian Randomization Study of the Causal Relationship Between Asthma-related Pneumonia and Heart Failure

  • 摘要:目的 采用双向两样本孟德尔随机化(Mendelian randomization,MR)分析探讨哮喘相关性肺炎与心力衰竭(Heart failure,HF)之间的因果关系。方法 本研究基于公开的全基因组关联研究(genome-wide association study,GWAS)数据,通过两样本MR分析,采用逆方差加权(Inverse Variance Weighted,IVW)、MR-Egger、加权中位数、加权众数和简单众数方法,系统评估哮喘相关性肺炎与HF的因果关系。使用不同的模型和检验方法进行敏感性分析,评估结果的稳健性。结果 根据IVW分析的结果,哮喘相关性肺炎对HF风险存在因果关系(OR=1.113,95%CI:1.027-1.206,P=0.009)。同时,根据IVW分析,HF对哮喘相关性肺炎风险不存在因果关系(OR=1.018,95%CI:0.780-1.328,P=0.893)。所有MR分析中使用的工具变量(Instrumental Variables,IVs)均未显示出异质性或水平多效性。此外,留一法分析显示,没有任何单个单核苷酸多态性(Single Nucleotide Polymorphism, SNP)对结果产生不成比例的影响,进一步证实了双向MR结果的可信度。结论 本研究确定了哮喘相关性肺炎对HF风险存在因果关系。然而,尚未发现HF对哮喘相关性肺炎风险存在明确的因果关系。该发现可为伴有HF的哮喘相关性肺炎患者的有效管理提供治疗指导。

    Abstract: Objectives To explore the causal relationship between Asthma-related pneumonia and Heart failure (HF) by using bidirectional two-sample Mendelian randomization (MR) analysis. Methods This study systematically evaluated the causal relationship between asthma-related pneumonia and HF through a two-sample MR analysis based on publicly available genome-wide association study (GWAS) data, employing Inverse Variance Weighted (IVW), MR-Egger, weighted median, weighted mode, and simple mode methods. Sensitivity analyses using different models and testing approaches were conducted to assess the robustness of the results. Results According to the results of IVW analysis, there is a causal relationship between Asthma-related pneumonia and HF risk (OR=1.113, 95%CI: 1.027~1.206, P=0.009). On the other hand, HF was not found to have a causal effect on Asthma-related pneumonia risk based on IVW analysis (OR=1.018, 95%CI: 0.780~1.328, P=0.893). The IVs used in all MR analyses showed no heterogeneity or horizontal pleiotropy. Moreover, the leave-one-out analysis revealed that no single SNP disproportionately influenced the results, further confirming the credibility of the bidirectional MR findings. Conclusion Through this study, it has been established that there is a causal relationship between pneumonia related to Asthma and the risk of HF. However, no definitive causal link has been found from HF to Asthma-related pneumonia. These insightful findings provide guidance for effectively managing the health of Asthma-related pneumonia with HF.
