Analysis of Clinical Medication Patterns for Lumbar Arthralgia Syndrome Based on Data Mining Technique

• 摘要：目的：基于数据挖掘技术分析腰痹病的临床用药规律。方法：收集湖南中医药大学第二附属医院谢教授门诊病历199则，处方199首，涉及中药149味，运用古今医案云平台（V2.3.5）进行药物频次、属性、归经、关联规则及聚类分析。结果：高频药物包括当归、盐杜仲、川芎等；药性以温（22.05%）、平（17.02%）为主，药味以甘（36.7%）、辛（25.14%）、苦（23.5%）为主；归经多属肝（30.22%）、脾（28.66%）、肾（27.25%）经。核心功效集中于强筋骨、补肝肾、活血止痛。关联规则分析获得46组常用药对，聚类分析提取4类核心药组。结论：谢教授治疗腰痹病强调以补肝肾、强筋骨治其本，以活血止痛、通经活络治其标，体现了其整体调节与标本兼治的学术原则。

  Abstract: Objective:To analyze the clinical medication patterns for lumbar arthralgia syndrome (Bi syndrome) using data mining techniques.Methods:A total of 199 outpatient medical records and corresponding prescriptions (involving 149 distinct Chinese herbal medicines) from Professor Xies clinic at the Second Affiliated Hospital of Hunan University of Chinese Medicine were collected. The Ancient and Modern Medical Case Cloud Platform (V2.3.5) was employed to conduct analyses on herb frequency, properties, meridian tropism, association rules, and clustering.Results:The high-frequency herbs included Dang Gui (Angelicae Sinensis Radix), Yan Du Zhong (Eucommiae Cortex Praeparata Cum Sal), and Chuan Xiong (Chuanxiong Rhizoma). Herb properties were predominantly warm (22.05%) and neutral (17.02%). The main herb flavors were sweet (36.7%), pungent (25.14%), and bitter (23.5%). The primary meridian tropisms were the Liver (30.22%), Spleen (28.66%), and Kidney (27.25%) meridians. The core therapeutic effects focused on strengthening bones and muscles, tonifying the Liver and Kidney, and promoting blood circulation to relieve pain. Association rule analysis identified 46 groups of commonly used herb pairs, and cluster analysis extracted 4 core herb groups. Conclusion: Professor Xies treatment for lumbar arthralgia syndrome emphasizes treating the root cause by tonifying the Liver and Kidney and strengthening bones and muscles, while addressing the symptoms by promoting blood circulation to relieve pain and activating meridians and collaterals. This approach reflects his academic principle of holistic regulation and simultaneous treatment of both the root and symptoms.
