The role of professional identity as a mediator between the clinical learning environment and clinical practice competence of masters degree nursing students

• 摘要：目的 通过调查我国护理专业学位硕士(Master of Nursing Specialist,MNS)研究生临床实践能力现状,发现在培养MNS研究生过程中存在的不足,为完善高等医学院校MNS研究生临床实践培养方案提供依据,为推动我国MNS研究生教育的发展提供参考。方法 采用便利抽样法,选取西北地区多所医学院校的207名全日制护理专业学位硕士研究生,使用一般资料问卷、临床学习环境评价量表、护士职业认同量表和临床实践能力问卷对其进行问卷调查。采用Spearman分析各变量间的相关性；Bootstrap法分析职业认同在临床学习环境与临床实践能力的中介作用。结果 MNS研究生临床实践能力总分为(210.03±27.29)分、临床学习环境总分为(139.39±20.43)分、职业认同总分为(115.54±17.23)分,Spearman分析显示三者呈正相关(P＜0.05)。采用Bootstrap法进行中介效应检验,结果表明职业认同在临床学习环境与临床实践能力之间发挥完全中介作用。结论 MNS研究生临床实践能力处于中上水平,优质的临床教学环境、高效的临床学习氛围可使MNS研究生真正的认可和接纳自己的职业,从而提升其临床实践能力。

  Abstract: Objective To investigate the current status of clinical practice ability of Master of Nursing Specialist (MNS) postgraduates in China, to find out the deficiencies in the process of training MNS postgraduates, to provide a basis for improving the clinical practice training programme of MNS postgraduates in higher medical schools, and to provide a reference for the promotion of the development of postgraduate education of MNS in China.Methods A convenience sampling method was used to select 207 full-time masters degree nursing postgraduates from a number of medical schools in the Northwest China region, and questionnaires were administered to them using a general information questionnaire, a clinical learning environment evaluation scale, a nurses professional identity scale, and a questionnaire on clinical practice ability. Spearmans analysis was used to analyse the correlation between variables; Bootstrap method was used to analyse the mediating role of professional identity in clinical learning environment and clinical practice competence. Results MNS postgraduate students clinical practice competence total score was (210.03±27.29), clinical learning environment total score was (139.39±20.43), and occupational identity total score was (115.54±17.23), and Spearmans analysis showed that all three were positively correlated (P<0.05). Bootstrap method was used to test the mediating effect, and the results showed that occupational identity played a fully mediating role between clinical learning environment and clinical practice competence. Conclusion The clinical practice ability of MNS postgraduates is at an intermediate to advanced level, and a high-quality clinical teaching environment and an efficient clinical learning atmosphere can make MNS postgraduates truly recognise and accept their profession, thus enhancing their clinical practice ability.
