Abstract: D-Amino acids (D-AAs), as an important class of chiral small molecules, exhibit metabolic characteristics distinct from those of L-amino acids and demonstrate dynamic changes closely associated with the occurrence and progression of various diseases. In recent years, with the deepening of research on chiral metabolism, the potential role of D-AAs in kidney diseases has gradually gained attention. Particularly in chronic glomerulonephritis (CGN), the variations in D-AA levels have been correlated with renal function status and disease progression. This paper reviews the biological characteristics of D-AAs and their enantioselective metabolic features, summarizes recent advancements in the study of D-AAs in CGN and related diseases, and highlights their potential value in evaluating renal function, screening biomarkers, and cross-disease applications. Additionally, the role of artificial intelligence and high-throughput detection technologies in advancing D-AA research is discussed. Finally, the mechanisms of D-AAs in kidney diseases, their clinical translational prospects, and future research directions are explored, aiming to provide insights for early diagnosis, prognostic evaluation, and precision intervention in CGN.
