Abstract: Objective: To explore the diagnostic value of olfactory testing, transcranial ultrasound (TCS) combined with serum neuroprotective associated protein 4 (NPADP-4), regulated on activation, normal T cell expressed and secreted (RANTES) detection for early Parkinsons disease (PD), and provide reliable evidence for early clinical identification of PD.Method: 138 early PD patients admitted to the neurology department of our hospital from January 2021 to December 2023 were selected as the PD group, and 136 healthy individuals who underwent health examinations at our hospital during the same period were selected as the control group. All research subjects completed olfactory testing, transcranial ultrasound examination, and serum NPADP-4 and RANTES level detection. Compare the differences in various detection indicators between two groups, analyze the diagnostic efficacy of individual indicators and combined detection for early PD using the receiver operating characteristic curve, and calculate the area under the c
