    Abstract: Objective To use a self-developed AI intelligent portable digital device to screen the oral health status of primary school students in remote mountainous areas of Chongqing, assess the prevalence of dental caries among children in the region, and explore childrens oral health and screening strategies. Methods A total of 106 children aged 7-13 from the Xianren Lake Central Primary School in Fengdu County, Chongqing, were selected as study subjects through cluster sampling. The AI portable device was used to conduct a comprehensive oral health examination of the participants. The prevalence of caries, number of decayed teeth, and number of affected tooth surfaces were reported; International Caries Detection and Assessment System Ⅱ (ICDAS Ⅱ), decayed, missing and filled teeth (dmft), and Plaque index (PLI) scores were calculated; and tooth surface and tooth position subgroup analyses was conducted. Results The overall caries prevalence rate in the school was 93.40%. The PLI score was 2.30 ± 0.64, and the dmft index was 6.45±4.38. The proportion of cavitated caries was higher than that of early-stage caries in both tooth surfaces and tooth positions, with the occlusal surfaces having the highest caries prevalence. The dmft index was significantly higher in younger children (7–9 years) compared to older children (10–13 years) (P = 0.002). Conclusion The caries prevalence rate among school-aged children in remote mountainous areas of Chongqing is extremely high, indicating a severe oral health status. AI-powered portable devices should be utilized to expand the scope of screening, increase screening frequency, and enhance caries prevention and treatment efforts for younger children.
