Effects of Shugan Huoxue Yishen decoction on homocysteine, inflammatory factors and endothelial function in patients with metabolic syndrome-related erectile dysfunction

• 摘要：目的 观察疏肝活血益肾方对代谢综合征（metabolic syndrome，MS）相关勃起功能障碍(erectile dysfunction，ED)患者血清同型半胱氨酸(homocysteine，Hcy)、炎性因子和内皮细胞功能的影响。方法 搜集 2023年9月至 2024年12月南京中医药大学泰州附属医院就诊的91例代谢综合征相关勃起功能障碍患者，采用随机数字表法将其分为对照组45例和观察组46例。对照组按需口服他达那非治疗，观察组用疏肝活血益肾方联合他达那非按需口服治疗，治疗12周。观察两组治疗前后临床疗效、中医证候评分、国际勃起功能指数5（International Index of Erectile Function 5,IIEF-5）评分及血清同型半胱氨酸（Hcy）、超敏C反应蛋白(high sensitivity C-reactive protein，hs-CRP)、肿瘤坏死因子-α（tumor necrosis factor-α,TNF-α）、白介素-6(interleukin-6，IL-6)、一氧化氮（nitric oxide，NO）、内皮素-1（endothelin-1,ET-1）水平变化。观察两组不良反应发生情况。结果 治疗12周后，观察组临床疗效优于对照组（P＜0.05）；观察组中医证候评分低于对照组（P＜0.05），IIEF-5评分高于对照组（P＜0.05）；观察组Hcy、hs-CPR、TNF-α、IL-6水平低于对照组（P＜0.05），观察组NO水平高于对照组，ET-1低于对照组（P＜0.05），两组不良反应比较，差异无统计学意义（P＞0.05）。结论 疏肝活血益肾方可降低MS相关ED患者血清同型半胱氨酸和炎性因子水平、改善内皮功能，改善患者的勃起功能、提高临床疗效，具有临床应用价值。

  Abstract: Objective To observe the effect of Shugan Huoxue Yishen decoction on serum homocysteine(Hcy),inflammatory factors and endothelial homocysteine function in patients with metabolic syndrome (MS) related erectile dysfunction dysfunction (ED) . Methods A total of 91 patients with metabolic syndrome related erectile dysfunction from September 2023 to December 2024 in Nanjing University of Chinese Medicine in Taizhou affiliated hospital were collected, the patients were randomly divided into control group (N = 45) and observation group (N = 46) . The patients in the control group were treated with oral administration of tadanafil as needed, while those in the observation group were treated with the combination of Shugan Huoxue Yishen decoction and tadanafil as needed for 12 weeks. The clinical efficacy, TCM syndrome score, International Index of Erectile Function 5(IIEF-5) score, serum homocysteine, high sensitivity C-reactive protein (hs-CRP) , tumor necrosis factor-α (TNF-α) , interleukin-6(IL-6) , nitric oxide (No) and endothelin-1(ET-1) levels were observed before and after treatment in the two groups. Adverse reactions were observed in both groups. Results After 12 weeks of treatment, the clinical effect of the observation group was better than that of the control group (p < 0.05) , the score of TCM syndrome in the observation group was lower than that of the control group (p < 0.05) , and the score of IIEF-5 was higher than that of the control group (p < 0.05) The levels of Hcy, hs-CPR, TNF-α and IL-6 in the observation group were lower than those in the control group (p < 0.05) . The levels of NO in the observation group were higher than those in the control group, while the levels of ET-1 were lower than those in the control group (p < 0.05). There was no significant difference in adverse reactions between the two groups (P > 0.05).Conclusion Shugan Huoxue Yishen decoction can decrease the levels of serum homocysteine and inflammatory factors, improve endothelial function, improve erectile function and clinical efficacy in patients with MS-associated ED, it has clinical application value
