摘要:肺炎支原体肺炎(mycoplasma pneumoniae pneumonia,MPP)是由感染肺炎支原体引起的肺部炎症,临床以发热、咳嗽为主要症状,较重者可出现持续高热、气促、发绀、喘息等。近年来,MPP在世界各地出现阶段性范围性爆发趋势,有关大环内酯类耐药的报道增多,众学者进行了调节免疫失衡治疗MPP的研究。中医通过清热化痰、益气养阴等治法可调节免疫失衡,降低炎症因子水平,西医联合小剂量糖皮质激素或丙种球蛋白抑制过度炎症反应。二者协同干预可更精准调控,改善免疫紊乱状态,从而缩短病程、减少重症转化风险。本文从发病机制、临床应用等角度,综述中西医疗法如何调节免疫失衡,为MPP治疗提供新的研究方向。
Abstract: s? Mycoplasma pneumoniae is a kind of lung inflammation caused by infection with Mycoplasma pneumoniae, which can involve bronchi, fine bronchioles, alveoli and interstitium of the lungs, and the clinical manifestations mainly include fever and cough. In more severe cases, persistent high fever, shortness of breath, cyanosis and wheezing may occur. In recent years, Mycoplasma pneumonia has shown a tendency to stage-wide outbreaks around the world, and reports about macrolide resistance have increased, and many scholars have conducted a series of studies on the regulation of immune imbalance by Chinese and Western medicine for the treatment of paediatric drug-resistant Mycoplasma pneumonia. In this paper, we review how Chinese and Western medical therapies regulate the immune imbalance from three perspectives: the pathogenesis of Mycoplasma pneumoniae, the mechanism of drug resistance, and the regulation of immune imbalance, so as to provide a new research direction for the treatment of paediatric drug-resistant Mycoplasma pneumonia.
