摘要:急性肺损伤(acute lung injury,ALI)及其严重类型急性呼吸窘迫综合征(acute respiratory distress syndrome,ARDS)是临床常见的危重症综合征。其发病机制复杂,与炎症反应、氧化应激、Ca2?稳态失衡及细胞损伤等因素密切相关。肺泡-毛细血管屏障是维持肺内气体交换和液体稳态的重要结构,其损伤可导致血管通透性增加、肺水肿及顽固性低氧血症,是ALI/ARDS发生发展的关键环节。目前临床治疗仍以呼吸支持和对症处理为主,针对屏障保护和修复的靶向治疗仍较缺乏。本文综述肺泡-毛细血管屏障的结构特点、损伤机制及防治研究进展,以期为急性肺损伤的临床治疗提供参考。
Abstract: Acute lung injury (ALI) and its more severe form, acute respiratory distress syndrome (ARDS), are common critical clinical syndromes. Their pathogenesis is complex and is closely associated with inflammatory responses, oxidative stress, Ca2? homeostasis imbalance, and cellular injury. The alveolar–capillary barrier is a crucial structure for maintaining pulmonary gas exchange and fluid homeostasis. Its disruption leads to increased vascular permeability, pulmonary edema, and refractory hypoxemia, representing a key event in the initiation and progression of ALI/ARDS. Currently, clinical management mainly relies on respiratory support and symptomatic treatment, while targeted therapies for barrier protection and repair remain limited. This review summarizes the structural characteristics of the alveolar–capillary barrier, the mechanisms of its injury, and recent advances in prevention and treatment, aiming to provide references for the clinical management of acute lung injury.
