Smart Elderly Health Care and Elderly-only Family Medical Expenditure

• 摘要：目的：评估智慧健康养老政策对降低纯老家庭医疗费用支出的实际效果。方法：利用中国老年社会追踪调查（CLASS）2014-2023年的五期数据，构建渐进式双重差分模型,检验政策前后纯老家庭医疗费用支出是否存在显著变化；结果：政策显著降低了纯老家庭的医疗费用支出，在控制相关变量后，试点使该类家庭的医疗费用支出平均下降1.13%，且通过了多项稳健性检验。机制分析表明，政策的降费效果主要是通过两条路径实现。一是慢病监测与预防，即通过提高疾病早期的识别率降低慢病共病及重症化的发生率；二是增加体育锻炼，政策配套设施提升了老年人的运动频率，增强了其健康资本。结论：智慧健康养老政策能够能够有效缓解纯老家庭的医疗经济负担。建议扩大政策覆盖范围，强化政策意向性效应，完善数字化预防体系，推动医体协同融合，并增加适老化情感支持。关键词：智慧健康养老、纯老家庭、医疗费用支出、渐进式双重差分模型；

  Abstract: Objective: To evaluate the impact of smart elderly care policies on the medical expenditures of elderly-only households.Methods: Based on 2014-2013 data from the China Longitudinal Aging Social Survey, this study employs a staggered difference-in-differences model to identify policy-induced variations in medical spending.Results: The policy reduces medical expenses for target households. After controlling for covariates, pilot led to an average 1.13% decrease in expenditures, a finding robust to multiple specifications. Mechanism analysis reveals two primary pathways: (1) chronic disease prevention, which lowers comorbidity and severity risks through early identification; and (2) health capital accumulation, driven by increased exercise frequency enabled by policy-supported facilities.Conclusion: Smart healthcare initiatives effectively mitigate the medical financial burden on elderly-only households.Efforts should focus on expanding coverage, leveraging intention-to-treat effects, and optimizing digital preventive systems. Furthermore, fostering medical-sports integration and enhancing emotional support for seniors are essential for maximizing policy effectiveness.
