• 摘要：目的观察发酵乳杆菌 CECT5716 联合阳和汤治疗阴证非哺乳期乳腺炎(NLM)的临床疗效。方法 选取 2023 年 4 月至 2024 年 12 月浙江中医药大学附属第一医院收治的阴证非哺乳期乳腺炎患者 42 例作为研究对象,联合组 22 例采用发酵乳杆菌 CECT5716 联合阳和汤治疗,中药组 20 例采用单纯阳和汤治疗,12 周为 1 个疗程。比较两组患者的 SF-36 生活质量量表评分、中医证候评分、肉芽肿性小叶性乳腺炎疾病活动指数(GDMAI)及治疗有效率。结果 治疗后两组患者的 SF-36 生活质量评分、中医证候评分、GDMAI 均较治疗前明显改善,且联合组患者的躯体健康所致角色限制(RP)评分、社交功能(SF)评分、中医证候得分减少百分比及肿块质地改善程度较中药组更显著(P<0.05)。治疗后,联合组患者的治疗有效率(73.0%)明显高于中药组(60.0%),差异有统计学意义(P<0.05)。结论 发酵乳杆菌 CECT5716 联合阳和汤治疗可显著改善阴证非哺乳期乳腺炎患者的生活质量,减轻临床症状,提升治疗有效率,效果优于单纯中药治疗,值得临床推广应用。

  Abstract: Objective: To observe the clinical efficacy of Lactobacillus fermentum CECT5716 combined with Yanghe Decoction in the treatment of Yin syndrome non-lactating mastitis (NLM). Methods: A total of 42 patients with Yin syndrome non-lactating mastitis admitted to The First Affiliated Hospital of Zhejiang Chinese Medical University from April 2023 to December 2024 were selected as the research objects. They were divided into two groups: the combined group (22 cases) treated with Lactobacillus fermentum CECT5716 combined with Yanghe Decoction, and the TCM group (20 cases) treated with Yanghe Decoction alone. A 12-week period was set as one course of treatment. The SF-36 Quality of Life Scale scores, TCM syndrome scores, Granulomatous Lobular Mastitis Disease Activity Index (GDMAI), and treatment effective rate were compared between the two groups. Results: After treatment, the SF-36 Quality of Life Scale scores, TCM syndrome scores, and GDMAI of patients in both groups were significantly improved compared with those before treatment. Moreover, the Role Limitations due to Physical Health (RP) scores, Social Functioning (SF) scores, percentage reduction of TCM syndrome scores, and improvement of lump texture in the combined group were more significant than those in the TCM group (P<0.05). After treatment, the treatment effective rate of the combined group (73.0%) was significantly higher than that of the TCM group (60.0%), and the difference was statistically significant (P<0.05). Conclusion: Lactobacillus fermentum CECT5716 combined with Yanghe Decoction can significantly improve the quality of life, alleviate clinical symptoms, and increase the treatment effective rate of patients with Yin syndrome non-lactating mastitis. Its effect is better than that of TCM treatment alone, and it is worthy of clinical promotion and application.
