• 摘要：目的 探究胃肠外科手术视频案例与术前讨论联合教学模式对医学生实习期间手术认知能力的影响。方法 选取2023年9月至2024年8月在山东第二医科大学附属医院胃肠外科实习轮转的60名医学生作为研究对象，采用随机数字表法将其分为对照组和观察组各30名。对照组采用传统带教模式进行实习指导；观察组在传统模式基础上增加手术视频案例学习与术前讨论环节。实习结束后对两组学生的手术理论知识、操作技能掌握程度及临床思维能力进行评估对比。结果 观察组医学生在手术认知评估总分、手术步骤识别、术中解剖标志辨识、并发症预判及手术配合能力评分均高于对照组（P<0.05）；观察组学生对教学满意度评分显著优于对照组（P<0.01）。结论 将胃肠外科手术视频案例与术前讨论结合应用于实习生教学中，能够有效提升医学生对手术流程的认知能力和临床思维水平，值得在医学教育实践中推广应用。

   

  Abstract: Objective To investigate the impact of a combined teaching model of gastrointestinal surgery video cases and preoperative discussions on the surgical cognitive abilities of medical students during their internship. Methods A total of 60 medical students rotating through the gastrointestinal surgery department at Shandong Second Medical University Hospital from September 2023 to August 2024 were selected as research subjects. Using a random number table method, they were divided into a control group and an observation group, with 30 students in each. The control group received traditional teaching methods, while the observation group received additional surgical video case studies and preoperative discussion sessions based on the traditional model. After completing the internship, both groups were evaluated on their theoretical surgical knowledge, operational skills mastery, and clinical thinking abilities. Results The observation group scored significantly higher in surgical cognitive assessment total scores, surgical step recognition, intraoperative anatomical landmark identification, complication prediction, and surgical cooperation ability compared to the control group (P<0.05). The teaching satisfaction scores of the observation group were significantly better than those of the control group (P<0.01). Conclusion Integrating gastrointestinal surgical video cases with preoperative discussions in intern teaching can effectively enhance medical students cognitive abilities regarding surgical procedures and clinical thinking skills, making this approach worthy of promotion in medical education practice.
