A Study on the Influencing Factors of Rehabilitation Outcomes in Patients with Acute Ischemic Stroke Complicated with Hypertension　LI Youfan1,LI Qiqi1,WANG Shangshu2,HAO Xijun1*,GUO Quanrong1

• 摘要：目的　探讨急性缺血性脑卒中（Acute ischemic stroke，AIS）伴高血压患者早期不同认知功能亚型受损对康复结局的影响。方法　采用前瞻性队列研究设计，连续纳入2024年8-12月于华北理工大学附属医院及开滦总医院（华北理工大学第二临床医学院）的280例AIS伴高血压患者。入院后72 h内评估蒙特利尔认知评估量表（Montreal Cognitive Assessment Scale，MoCA）各维度得分及美国国立卫生研究院卒中量表（National Institute of Health Stroke Scale，NIHSS）总分；3个月随访采用扩展格拉斯哥预后量表（Glasgow outcome scale—extend，GOS-E）评估康复结局。采用Spearman秩相关、Mann-Whitney U 检验及多元线性回归分析数据。结果　多元线性回归显示,高血压分级（β=-0.093,P<0.05）、NIHSS总分（β=-0.278,P<0.001）、MoCA语言维度(β=0.299,P<0.001)、MoCA抽象维度(β=0.170,P<0.05)是AIS伴高血压患者康复结局的独立影响因素。结论　AIS伴高血压患者早期语言和抽象认知功能受损是康复结局的关键预测指标，提示针对性认知康复干预可优化预后。

  Abstract: Objective　To investigate the impact of early impairment in different cognitive domains on rehabilitation outcomes in patients with acute ischemic stroke (AIS) and hypertension. Methods　A prospective cohort study was conducted, consecutively enrolling 280 AIS patients with hypertension from the Affiliated Hospital of North China University of Science and Technology and the Kailuan General Hospital between August and December 2024. Within 72 hours of stroke onset, cognitive function was assessed using domain scores of the Montreal Cognitive Assessment (MoCA), and stroke severity was measured using the National Institutes of Health Stroke Scale (NIHSS) score. Rehabilitation outcome was evaluated at the 3-month follow-up using the Glasgow Outcome Scale-Extended (GOS-E, score range 1-8). Data analysis employed Spearman rank correlation, Mann-Whitney U test, and multiple linear regression. Results　Multiple linear regression analysis revealed that hypertension grade (β = -0.093, P < 0.05), NIHSS total score (β = -0.278, P < 0.001), MoCA language domain score (β = 0.299, P < 0.001), and MoCA abstraction domain score (β = 0.170, P < 0.05) were independent influencing factors of rehabilitation outcomes in AIS patients with hypertension. Conclusion　Early impairment in the language and abstraction cognitive domains significantly affects rehabilitation outcomes in AIS patients with hypertension.
