Abstract: Diabetes mellitus is a highly prevalent chronic metabolic disorder globally. Particularly, Type 2 Diabetes Mellitus (T2DM) is a complex metabolic disease characterized by insulin resistance (IR) and progressive functional impairment of pancreatic β-cells as its core pathophysiological features. Its pathogenesis involves interactions among multiple organs and systems and is closely associated with a chronic low-grade inflammatory state. Although traditional glucose-lowering drugs are effective, long-term use may be accompanied by side effects or economic burdens. In recent years, the natural compound berberine has garnered significant attention due to its multi-target regulatory effects on blood glucose. This article systematically reviews the multi-target mechanisms by which berberine regulates blood glucose and summarizes its research progress in the prevention and treatment of diabetes. The aim is to provide a theoretical basis and innovative insights for the development of glucose-lowering drugs based on natural compounds.
