Progress in diagnosis and treatment of deep vein thrombosis after trauma

• 摘要：【】 创伤(Trauma)是导致年轻人死亡和残疾的主要原因,而静脉血栓栓塞症(venous thromboembolism, VTE)是创伤后常见的并发症,也是导致患者死亡的重要原因。静脉血栓栓塞症(VTE)主要包括肺栓塞(pulmonary embolism, PE)和深静脉血栓(deep vein thrombosis, DVT)。创伤患者由于长期卧床、凝血异常以及其他多种危险因素,DVT的风险显著增加。血栓前的预防及血栓后及时的诊断和治疗可降低该患者群体的血栓风险,从而降低创伤患者住院期间的死亡率。创伤患者的凝血功能处于从低凝到高凝的波动状态,临床医师能否准确动态地评估患者的血栓风险,并及时给予适当的处理和预防仍是目前临床所面临的关键问题。本文将从DVT的发病机制、临床诊断、治疗及预防等方面展开综述。

   

  Abstract: 【】 Trauma is a leading cause of death and disability among young individuals, with venous thromboembolism (VTE) being a common post-traumatic complication and a significant contributor to mortality. VTE primarily encompasses pulmonary embolism (PE) and deep vein thrombosis (DVT). Due to long-term bedridden, coagulation abnormalities, and other risk factors, trauma patients face a markedly increased risk of DVT. Prophylaxis for thrombosis and timely diagnosis and treatment can significantly reduce the incidence of thrombosis in these patients, thereby decreasing mortality during hospitalization. The coagulation status of trauma patients alternates between hypocoagulable and hypercoagulable states. The ability of clinicians to accurately and dynamically assess the thrombotic risk of patients and provide timely and appropriate management and prevention remains a critical concern in contemporary clinical practice. This article reviews the pathogenesis, clinical diagnosis, treatment, and prevention of DVT.
