中医外治技术改善化疗后骨髓抑制的网状Meta分析

Efficacy of Traditional Chinese Medicine External Therapies for Chemotherapy-Induced Myelosuppression: A Network Meta-Analysis

摘要:目的 采用网状Meta分析方法评价不同中医外治技术干预化疗后骨髓抑制的效果。方法 计算机检索中国知网、万方、维普、中国生物医学文献数据库、PubMed、Cochrane图书馆、EMbase和Web of Science数据库,检索年限为建库至2025年11月,检索纳入不同中医外治技术干预化疗后骨髓抑制的随机对照试验。由2名研究者独立进行文献筛选、质量评价和资料提取后,采用R4.2.2软件进行网状Meta分析。结果 共纳入29篇文献,总样本量2292例(试验组1151例、对照组1141例),涉及17种中医外治技术。结果显示:在提升白细胞计数方面,雷火灸(SUCRA=0.92),穴位注射联合艾灸(SUCRA=0.75),穴位注射(SUCRA=0.74)疗效最优;改善中性粒细胞计数以穴位注射(SUCRA=0.96),麦粒灸(SUCRA=0.76),穴位注射联合艾灸(SUCRA=0.64)效果最佳;提升血红蛋白的最优方案为穴位注射联合艾灸(SUCRA=0.88),隔药灸(SUCRA=0.86),雷火灸(SUCRA=0.64);改善血小板计数以雷火灸(SUCRA=0.97),穴位注射联合艾灸(SUCRA=0.80),麦粒灸(SUCRA=0.73)表现突出。结论 中医外治技术可有效改善化疗后骨髓抑制,中药穴位注射与雷火灸技术推荐为首选干预技术,临床可根据患者具体血液指标异常情况选择个体化方案。本研究因纳入文献的数量及质量的原因,仍需更多高质量、大样本、长期随访的随机对照试验进一步验证。
Abstract: Objective: To evaluate the effects of different traditional Chinese medicine (TCM) external therapies on post-chemotherapy myelosuppression by network Meta-analysis.Methods:CNKI,WanFang Data,VIP, SinoMed, PubMed, the Cochrane Library,EMbase,and Web of Science were electronically searched to identify randomized controlled trials (RCTs) on different TCM external therapies for post-chemotherapy myelosuppression, with the retrieval period ranging from database establishment to November 2025. Two researchers independently performed literature screening, quality assessment, and data extraction, and R 4.2.2 software was used for network Meta-analysis. Results:A total of 29 literatures were included, involving 2292 participants (1151 in the experimental group and 1141 in the control group) and covering 17 types of TCM external therapies. The results showed that for increasing white blood cell count, Thunder Fire Moxibustion (SUCRA=0.92), acupoint injection combined with moxibustion (SUCRA=0.75), and acupoint injection (SUCRA=0.74) had the optimal efficacy; for improving neutrophil count, acupoint injection (SUCRA=0.96), grain-sized moxibustion (SUCRA=0.76), and acupoint injection combined with moxibustion (SUCRA=0.64) performed the best; the optimal schemes for increasing hemoglobin were acupoint injection combined with moxibustion (SUCRA=0.88), medicine-separated moxibustion (SUCRA=0.86), and Thunder Fire Moxibustion (SUCRA=0.64); and for improving platelet count, Thunder Fire Moxibustion (SUCRA=0.97), acupoint injection combined with moxibustion (SUCRA=0.80), and grain-sized moxibustion (SUCRA=0.73) were outstanding. Conclusions:TCM external therapies can effectively improve post-chemotherapy myelosuppression, with TCM acupoint injection and Thunder Fire Moxibustion recommended as the preferred intervention techniques; individualized regimens can be selected clinically based on patients specific abnormal blood indicators. Due to the quantity and quality of the included literatures, more high-quality, large-sample, and long-term follow-up RCTs are required for further verification.
